Consent and Release
By attending The Orthobiologics Forum (the “Event”), I acknowledge and agree that photographs, video recordings, and other media may be captured during the Event.
I hereby grant the CAO Research Foundation, its affiliates, representatives, and authorized partners the irrevocable right and permission to photograph, record, and use my name, image, likeness, voice, and professional identity (collectively, “Likeness”) in any media now known or hereafter developed.
Permitted Use
I understand and agree that such materials may be used for legitimate purposes including, but not limited to:
Educational and scientific programming
Marketing and promotional materials
Website, social media, and digital communications
Event recaps, publications, and future program development
All uses will be in a professional and non-defamatory manner consistent with the mission of the CAO Research Foundation.
No Compensation
I acknowledge that I will receive no compensation, royalties, or other consideration for the use of my Likeness.
Waiver of Claims
I waive any right to inspect or approve the final materials and release the CAO Research Foundation and its representatives from any and all claims, demands, or liabilities arising out of or related to the use of my Likeness, including but not limited to claims for:
Defamation
Invasion of privacy
Right of publicity
Voluntary Participation
My attendance at the Event is voluntary, and this release is given freely without inducement.
Opt-Out (Optional – include if you want flexibility)
If I do not wish to be photographed or recorded, I understand it is my responsibility to notify event staff in writing prior to or during the Event, and reasonable efforts will be made to accommodate my request.
Acknowledgment
By completing this online form, I acknowledge that I have read and understand this Photography & Likeness Release and agree to its terms.
